Sports Physicals

As always, our players need to have a new sports physical for each season.  Immanuel Schools is opening their south gym this Thursday at 5PM for all Immanuel student athletes to complete their sports Physicals and they have agreed to open this event to ALL Titan players regardless of their enrollment.  Cost will be $25 payable to “Immanuel Schools”.  Bring the attached form(Sports Physical Packet 2022) to be completed by the medical staff at this event.

If you are unable to attend this event, any form from your primary doctor will be sufficient, just make it clear that your athlete will be playing TACKLE FOOTBALL.  We must have a sports physical on file in order for an athlete to participate in our first practice on 7/25.
